Please note that the documents listed below need to be submitted in addition to the registration form to complete the registration process:

1. Proof of Birth - Birth Certificate or Similar Document
2. Proof of Identity
3. Medical Records – Immunization Records, Physical, Other
4. Proof of Residency
5. Last Report Card from Previous School (if applicable)
6. Custody/Legal Documents (if applicable)
